List of Requirements for Approval of Lumber Supply Contract

Request Letter attached with the following:

1. Authenticated copy of  business registration as single proprietorship, partnership or corporation from DTI or SEC (for new buyer only);
2. Copy of  permit ( supplier and buyer if both permittee);
3. Business Permit (new buyer only); and
4. Validated Monthly or Quarterly latest report of the supplier
