1. Duly accomplished application form
For renewal, application must be submitted at least two (2) months prior the expiration to give ample time for the processing and evaluation of the submitted documents
Note: Application filed after expiration shall be treated as new
2. Payment of Fees: per DAO 2004-16
- Application Fee – Php 600.00
- Permit Fee_ based on Daily Rated Capacity (DRC)
- Forestry Bond – 720/cu.m (based on DRC but not less than Php 6,000.00
3. Original or Certified True Copy of Certificate of Registration, Articles of Incorporation, Partnership of Corporation, as the case maybe (for new application)
4. Authorization issued by the Corporation, Partnership or Association in favor of the person signing the application
5. Feasibility Study (for new application)
6. Business Plan
7. Original or Certified True Copy Business permit issued by Municipality Mayor
8. Original or Certified True Copy of Environmental Compliance Certificate (ECC) issued by the Environmental Management Bureau (EMB) and all pertinent permits and requirements stipulated therein
9. Previously approved WPP Permit (for renewal)
10. Certificate of Good Standing to be executed by the concerned Regional Office, stating among others that the subject WPP has never been involved in any illegal logging activities in the past
11. Original Copy of Proof of sustainable sources of legally cut logs for a period of at least 5 years, supported by the following documents:
For Local Wood Raw Materials:
- Original copy of Log/Veneer/Lumber Supply Contracts duly approved by the concerned Regional Executive Director
- At least 5% Tree Inventory of the forest/private tree plantation that includes under oath, narrative report, tally sheets, stand and stock table, geotag photographs and map of the area
- Electronic copy of inventory data in MS Excel Format
- Under oath validation report of the PENRO/RO as to the availability and sustainability of the volume of raw materials covered by a Log Supply Contract duly approved by the RED
