List of Requirements for Permit to Sell, Re-sell, Dispose Distribute, Transfer of Ownership of Chainsaw (2022)

1. Duly accomplished application form  (1 original)

2. Request letter (1 original) with the following information:

• Number of chainsaw to be sold
• Specification of chainsaw (serial nos., model, brand, horsepower, etc.)

3. Proof of ownership of chainsaws (sales invoice, deed of sale, etc.)

4. Business Name Registration of applicant from DTI or SEC

5. Permit fee (Php 500.00)

6. Copy of Permit to  Import /Purchase
