2. LGU Certification of No Objection (1 original) (1 Duplicate)

3. Payment of Fees

1. Oath Fee (Php 36.00)
2. Inventory Fee (Php 1,200.00/ha for application involving 1 ha. and above)

4. Photographs of trees

5. OCT/TCT (1 certified copy or 1 photocopy with accompanying owner’s copy)

Additional if within subdivisions

6. Homeowner’s Association Certification of No Objection re: cutting of tree(s) (1 original copy)
