1. Duly accomplished application form

2. LGU Endorsement/Certification of No Objection (1 original) (1 Duplicate)

3. Payment of Fees:

1. Oath Fee (Php 36.00)
2. Inventory Fee (Php 1,200.00/ha for application involving 1 ha. and above)

4. Photographs of trees

Additional if within subdivisions

5. Homeowner’s Association Resolution re: no objection on the cutting, earth-balling and/or pruning of trees (1 original copy)

Additional if School/Organization

6. PTA Resolution or Resolution from any organized group of No Objection and Reason from Cutting (1 original)
